it makes heros do 7% more dmg and take 7% less dmg. by itself it isnt that much but when you get 2-3 people leaving it makes pretty big difference. Once had a pub game where the other team all raged except a system O.o , op system that dealt 28%ish more dmg and was tank on top of it. He could ult and kill any of us in one hit even though we had spell resist items.

Except in that scenario, as soon as you kill him once you have free reign of tower pushing.

To answer the question, it only applies the bonus on players who have an active hero at the time the buff is awarded.

 

So if someone quits during load screen, you don't get it even though it says you do.

You also don't get it if you haven't picked your hero yet, but teammates who have would get the bonus.
